SDN refers to the decoupling of the control plane and the data plane of the network, allowing for the central control of the network.
This separates the control plane (which makes decisions about routing the traffic) from the data plane (which forwards traffic according to the control plane's decisions). Which allows for a central SDN controller to manage the network, making it easier to configure, monitor, and optimize the network.
